- In the Waste Disposition section, indicate if foundry waste will be used as fill material per relevant regulations. Provide the names and permit numbers of the landfills where the waste is proposed to be disposed.
- Complete the Certification section by signing and dating the form. Ensure it is signed by an authorized representative. Include the profile number and certification number if applicable.
- Once all sections are completed, save the changes made to the form. You may download, print, or share it as needed.
