- Complete the form by filling out your communication address, professional address (if applicable), telephone numbers, and email address, ensuring all information is accurate.
- Affirm the truthfulness of the information provided in the application, sign the document, and include your name.
- Submit your application by either delivering it in person or sending it via post. If mailing, ensure all documents and payment are included.
