Work Injuries

At the end of each calendar year, Form 300-A must be completed and certified by a company executive as correct and complete and posted in the workplace where notices to workers are usually posted. It must be posted for three months, from February 1 until April 30.

Convert all times to 24 hour clock (military time): Convert am to hours. Next, Subtract the start time from the end time. Now you have the actual hours and minutes worked for the day. Finally to determined total wage, you will need to convert this to a decimal format.

An incidence rate of injuries and illnesses may be computed from the following formula: (Number of injuries and illnesses X 200,000) / Employee hours worked = Incidence rate.

The information in OSHA records makes employers more aware of the kinds of injuries and illnesses occurring in the workplace and the hazards that cause or contribute to them. When employers analyze and review the information in their records, they can identify and correct hazardous workplace conditions on their own.

The formula is: Total number of injuries and illnesses ÷ Number of hours worked by all employees x 200,000 hours = Total recordable rate. The 200,000 figure represents the hours that 100 employees would work during 40-hour weeks, 50 weeks per year.

Covered establishments are only required to electronically submit information from the OSHA Form 300A (Summary of Work-Related Injuries and Illnesses). The requirement to keep and maintain OSHA Forms 300, 300A, and 301 for five years is not changed by this Final Rule.

The data submission process involves four steps: (1) Creating an establishment; (2) adding 300A summary data; (3) submitting data to OSHA; and (4) reviewing the confirmation email. The secure website offers three options for data submission. One option will enable users to manually enter data into a web form.
