Type "Employment Application" in the "Templates on Office Online" box and click the "Search" button. Select "Employment Application 2-pp Online Form" in the "Search Results" list. Click the "Download" button, and an application will open as a new Word document.

On Jan. 31, 2020, USCIS published the Form I-9 Federal Register notice announcing a new version of Form I-9, Employment Eligibility Verification, that the Office of Management and Budget approved on Oct. 21, 2019.

An employee information form contains key information on employees that is used to keep a record of who worked for the company, their duration of employment, and in what role. It can also be used as an emergency contact information form in the event of any serious workplace injury.

U.S. Citizenship and Immigration Services (USCIS) publishes a paper I-9 Form and an electronic, fillable I-9 Form. The new version of the paper form has no changes from the prior version. The form is used to verify the identity and employment authorization of individuals hired for employment in the United States.
