To change your name, you will need: Your current driver's licence or Ontario Photo Card. A proof of name change document (the document you bring as proof of your name change must be in English, French or be accompanied by a written translation from an approved translator) adopted name. court order for adoption OR.

Ontario driver's name. Always starts with last name followed by first name followed by middle name(s). Last, first and middle names are separated by commas.

To make any changes to your Ontario Photo Card, including your address and height, you must visit a ServiceOntario centre. You do not need to bring anything other than your card.

Visit a driver's licence office in person All provincial driver's licences must be updated in person either at a driver's licensing office or at your provincial service office. If you attend a service office you can often update your health care card, voter enrolment and driver's licence all at the same time.

DRIVER'S LICENCE The licence number starts with the first letter of the last name e.g., Jane Q. Public's licence number starts with a ?P?. The 9th and 10th number (separated by a dash) are the same as the last two numbers of the person's year of birth. The last two numbers indicate the day of birth.
