DescriptionThis Anatomical Gift Donation designates which body parts and organs a person under eighteen years of age, with parental consent, wishes to donate at the time of death. To be effective, a document of gift must be signed by the donor in the presence of two (2) witnesses who shall sign the document in his presence. If a donor is under eighteen (18) years of age, one (1) of the witnesses MUST be the donor's parent or legal guardian. | |||||||||||||||||
