Description

An individual who is under the age of eighteen, but is at least sixteen years of age, may make an anatomical gift if the document of gift is signed by either parent or a guardian of the donor. This form designates the body parts and organs which a person at least 16 years of age, with parental consent, wishes to donate at the time of death.
